You have the ability to access your patient records online through our patient portal. From this portal you can access your exam notes and records. You can also print them out from here as well.
Your user name is your last name, first initial and your 5 digit patient record number, all as one word with no spaces. Your initial password is Password1. You will be prompted to change your password immediately when you login for the first time to create a personal unique password.
